Gary Densham was competing at the 2016 Circle K NHRA Winternationals in Pomona when the parachute of his racer failed to open and he careered into the safety net

Densham was checked and released by NHRA emergency services officials without injury, after his car ran through the sand trap at speed and into the safety net.

I love how the commentator sounds all like “Oh. How unfortunate.”

Something similar happen in Puerto Rico, the guy driving was going to be the first Puerto Rican to race in Nascar, but he was not as lucky, both brakes and parachute failed, dying on impact.
